“Introduction: The Brazilian Unified Health System (Sistema Único de Saúde, SUS) is a public, universal health system. In Brazil, the right to health is guaranteed by the Federal Constitution as a duty of the State, which includes access to intensive care unit (ICU) treatment. Access to ICU beds has long been a topic of ongoing debate within SUS due to prolonged waiting times until admission. Therefore, the primary objectives of this study are to evaluate the waiting time to ICU admission and its effect on in-hospital mortality among patients hospitalized in ICUs of public hospitals in the Federal District, Brazil. Methods: This is a prospective cohort study that will consecutively include all individuals aged ≥18 years with a request for ICU admission across the 15 ICUs of public hospitals in the Federal District, Brazil, from July 2023 to June 2025. Patients transferred to private hospitals, or those with requests for transfer to a public ICU originating from a private hospital, will be excluded. Daily propensity scores for ICU admission during follow-up will be constructed and adjusted for factors independently associated with ICU admission and in-hospital mortality. These scores will be used to analyze the association between time to ICU admission and in-hospital mortality. Preliminary Results: Data were extracted from the electronic medical record system used by the Bed Regulation Center of the Federal District State Health Department (Central de Regulação de Leitos, Secretaria de Estado de Saúde do Distrito Federal—SES-DF) for the study years. The dataset is currently being validated for the consistency of recorded diagnoses, priorities assigned by the SES-DF Bed Regulation Center, and the dates/times of ICU request and ICU admission.”
